This sub-section provides templates exclusively for use in the Court of Appeal for Ontario. Although the Court of Appeal is governed by the Rules of Civil Procedure and, therefore, uses many forms in common with the Ontario Superior Court, the templates on this page will be only used in the Court of Appeal.
